Your physical therapy prescription allows you the right
to choose the facility where you want to receive treatment. |

You do not need a prescription to be seen for your
first evaluation visit. If additional visits are necessary, we will
send your physician an evaluation note for a prescription for physical
therapy on your behalf. |

Our office staff processes all of your insurance paperwork
and claims at no charge to you. (Insurance will not pay for your visit
without a physician's prescription. Only the initial evaluation will
be charged.) |
